Application for study leave

This form should be completed and sent to Rachel Dalby for Dr Webster's approval.

Petrol claim form

This should be completed each month (on blue paper!) and sent to the address at the bottom of the claim form (with a copy of the form and receipts sent to Rachel Dalby). The form should be used for all standard petrol claims and public transport/parking costs. This excludes routine home to base journeys (which cannot be reclaimed) - but includes study days and travel to meetings within the normal working week. Please note: study leave must be agreed in advance using the appropriate form.

Audit forms

In order to maintain and improve the quality of the training programme, trainees should complete the audit forms below (as appropriate) and send these to Rachel Dalby for collation.

  1. Oxford Deanery Public Health Training Programme - Induction programme audit form (to be completed by all new trainees within the first month of joining the Oxford Deanery Public Health Training Scheme).

  2. Oxford Deanery Public Health Training - General training audit (to be completed by trainees at the end of every attachment in the training scheme).

  3. Oxford Deanery Public Health Training - Communicable Disease and Environmental Health induction programme audit (these questions relate to the induction prior to joining the on-call rota. This form is to be completed at the end of the first posting with a health protection team).
